SMS Text Messaging & Email Opt Out Form
Burbury Medical Centre is increasingly using digital methods, e.g. email and text messages, to communicate with patients/service users. We are supportive of text and emails being used where they can support the delivery of care.
This service will be used to send texts to patients regarding upcoming clinic appointments, and in the future to send requests to attend for repeat tests, speak to the doctor / nurse or book an appointment. We may also use this service to contact patients about health promotion.
We always strive to maintain confidentiality of your information and will continue to do so while using this system. To help us do this, it is important that you let us know if you change your mobile number or email address in the future.
If you do not wish to receive texts or emails from the practice, please fill out the below form.
